Grant Overview

South Dakota contains nine federally recognized tribal nations whose combined land area exceeds 5 million acres and whose residents experience diabetes prevalence rates 2.4 times the state average. Capacity gaps specific to the state include the absence of any Level I trauma center west of the Missouri River and the fact that 11 of the state's 66 counties lack a single licensed primary-care physician. Infrastructure constraints are compounded by winter road closures on state highways serving the Cheyenne River and Standing Rock reservations, where average winter ambulance transport times exceed four hours. Workforce constraints appear in the Indian Health Service's documented 35 percent vacancy rate for physicians on South Dakota reservations. Readiness requirements therefore mandate that applicants maintain formal agreements with tribal health departments and demonstrate ability to deliver services during months when state highway 212 and 63 are closed. Unlike Nebraska applications, South Dakota requires demonstration of extreme rural delivery capacity due to frontier county designation. Eligible entities must provide maps of proposed clinic sites relative to the nearest Indian Health Service facility and confirm generator capacity sufficient for 72-hour outages. Budgets may cover only culturally adapted health-education curricula, community-health-worker salaries, and vehicle maintenance for mobile clinics. Reporting must include hemoglobin A1c screening rates and immunization completion rates disaggregated by reservation.
