South Dakota’s target outcomes center on measurable reductions in preventable hospitalizations among American Indian residents, who comprise 9 percent of the population yet account for 34 percent of diabetes-related admissions according to 2022 state hospital discharge data. The state’s nine reservations span distances that routinely exceed 100 miles between Indian Health Service facilities.

These distances interact with seasonal road conditions and limited broadband to constrain follow-up care. Younger tribal members show higher rates of substance use disorders while elders experience elevated cardiovascular mortality compared with statewide figures. Outcomes are tracked through tribal health department dashboards rather than uniform state portals.

The funding mechanism allows medical trainees to embed for two-to-eight-week periods within tribally operated clinics to deliver health education sessions that integrate traditional dietary practices with contemporary chronic disease management. Trainees must document pre- and post-session knowledge assessments approved by tribal councils.

Successful proposals demonstrate coordination with the Great Plains Tribal Chairmen’s Health Board and adherence to tribal research review processes. Review criteria include evidence that at least half of participant contacts occur on reservation land rather than in Rapid City or Sioux Falls.

Unlike North Dakota placements, South Dakota applications require documented approval from the specific tribal nation hosting the trainee and submission of a data-sharing agreement that respects tribal sovereignty over health metrics.
