Grant Overview

South Dakota’s 884,000 residents are dispersed across 77 counties, with 60 percent classified as frontier. The state operates two hospital systems with hematopathology capability, both located east of the Missouri River. Investigators seeking to study leukemia incidence in American Indian communities on the nine reservations face additional distance barriers exceeding 150 miles to the nearest cytogenetic laboratory.

Mobile health units currently deployed for mammography and colorectal screening provide an existing platform that could be adapted for blood-based leukemia detection studies. However, these units lack cold-chain capacity for viable cell transport and do not currently collect biospecimens under IRB-approved protocols. Early-stage research projects must therefore budget for equipment retrofits and training of tribal health staff.

Grant support covers study-related services including courier contracts that maintain 4 °C transport from Pine Ridge and Rosebud to Sioux Falls within eight hours. Personnel funding can support a 0.5 FTE research coordinator based at the Aberdeen Area Indian Health Service to manage consent and follow-up. Limited travel allocations enable attendance at annual tribal health board meetings required for community approval.

Applications must include tribal resolution letters and evidence of prior successful specimen collection on reservation land. Projects are evaluated on the proportion of samples obtained from frontier zip codes rather than from the Sioux Falls or Rapid City urban areas. This requirement ensures datasets reflect the demographic and geographic distribution necessary for valid incidence estimates.
