South Dakota Mental Health Access Barriers for Families
South Dakota reports the highest ratio of children under five to licensed child psychologists in the United States, with 1,840 children per provider. This shortage is most acute in the 34 counties classified as frontier, where travel distances to the nearest mental health clinic exceed 90 minutes.
Families in these counties include agricultural operators subject to seasonal income volatility and members of nine federally recognized tribes whose service areas overlap reservation boundaries. Stigma around behavioral health services remains elevated; 2021 Behavioral Risk Factor Surveillance System data indicate that 37 percent of South Dakota adults in non-metro counties report that they would not seek professional help for anxiety or depression.
The funding targets this barrier by requiring projects to locate services within existing primary care clinics or Indian Health Service facilities rather than creating standalone centers. Partnerships must include at least one South Dakota Medicaid managed care organization and one tribal health program when the service area intersects reservation land.
Proposals must document how telehealth platforms will comply with state parity statutes and address bandwidth limitations documented by the South Dakota Broadband Office. Unlike North Dakota projects that focus on oil-patch counties, South Dakota reviewers prioritize applications covering the Missouri River corridor and the Pine Ridge and Rosebud service areas, where provider vacancy rates exceed 40 percent. All evaluation plans must use the state's required outcome measures reported through the South Dakota Family and Community Health database.
