South Dakota’s 66 counties include 34 with no practicing neurologist and broadband download speeds below 25 Mbps in 28 percent of rural census blocks. These infrastructure constraints limit both in-person specialist access and reliable tele-neurology for residents spread across 77,000 square miles. The state’s American Indian population, concentrated on nine reservations, experiences neurology referral wait times averaging 4.7 months according to Indian Health Service facility logs.
Early-career researchers applying for the three-year award must submit a capacity assessment that maps existing clinic square footage and telehealth equipment against South Dakota Department of Health licensing standards. The proposal must identify at least three clinics that currently lack dedicated examination rooms suitable for neurological testing and propose renovation budgets within the $300,000 ceiling.
Salary support is capped at 35 percent of the researcher’s institutional base because South Dakota state universities apply a uniform 32 percent fringe-benefit rate that cannot be waived. Remaining funds must therefore prioritize durable equipment such as portable EEG units or tablet-based cognitive batteries that remain at host clinics after the project ends.
Letters of commitment must come from South Dakota-licensed facilities and reference the state’s certificate-of-need statute, which requires advance notice for any new neurological service line. This procedural step is absent in several contiguous states and directly affects project timelines.
