South Dakota’s operational requirements center on formal tribal health agreements because 11 percent of the state’s population resides on nine federally recognized reservations where Indian Health Service facilities serve as the primary diagnostic entry point. These facilities currently operate without on-site molecular pathology capacity for most solid-tumor biomarkers.
Partnership protocols must therefore route specimens to the single CLIA-certified reference laboratory in Sioux Falls, creating average turnaround times of 11 days. Any proposal that omits a signed memorandum of understanding with at least two tribal health boards will fail operational review regardless of scientific approach.
The funding notice requires documentation of chain-of-custody procedures that satisfy both Indian Health Service regulations and South Dakota state laboratory licensing rules. Teams must also project monthly specimen volumes against the existing courier schedule that runs only twice weekly from reservations to Sioux Falls.
Review panels evaluate whether staffing plans account for the state’s 14 percent vacancy rate for certified laboratory technologists in non-metropolitan counties. Projects that rely on new hires without secured housing stipends or tele-mentoring arrangements have been declined in prior cycles.
Unlike submissions from states without large tribal land bases, South Dakota applications must demonstrate that proposed diagnostic algorithms have been reviewed by the relevant tribal institutional review board before the federal deadline. This pre-review step adds 60–90 days to the preparation timeline and cannot be bypassed.
