South Dakota aims to reduce unplanned hospitalizations among transition-age youth with complex conditions by 18 percent over five years, measured through the state’s all-payer claims database. Target counties include those along the Missouri River corridor where 34 percent of adolescents with diabetes or epilepsy reside more than 60 miles from an adult subspecialist. The transitional support track funds hypothesis-driven studies on care handoff protocols adapted to low population density settings.
American Indian youth on the nine reservations account for 29 percent of transition failures documented in 2022 University of South Dakota Center for Rural Health reports. White adolescents in the western counties experience comparable discontinuity rates driven by out-migration for college and limited adult provider availability. Both groups require protocols that accommodate seasonal travel patterns tied to agriculture and tourism employment.
Outcome evaluation must use the state’s existing telehealth utilization metrics rather than new data collection systems. Career-development awards require placement of emerging investigators at one of the four critical access hospitals participating in the South Dakota Transition Collaborative. Project grants receive higher scores when they incorporate tribal health department data-sharing agreements, reflecting the 11 percent of the pediatric population living on reservation land.
Unlike North Dakota or Nebraska applications, South Dakota requires explicit modeling of winter weather delays on transition appointment completion rates and contingency plans using existing state EMS interoperability standards.
