South Dakota home-based monitoring grants target measurable reductions in 30-day hospital readmissions among Medicare beneficiaries aged 65 and older residing in counties with populations under 10,000. The state’s target is a 12 percent relative reduction within 24 months, benchmarked against 2022 claims data from the South Dakota Association of Healthcare Organizations. Proposed technology must integrate with the state’s existing telehealth network that covers 52 of 66 counties.

These outcomes matter because South Dakota’s senior population density is highest in the central and western counties where the nearest hospital often exceeds 80 miles. Winter weather events close roads for an average of 11 days per year, delaying in-person follow-up. Claims analysis shows that seniors in these areas experience 1.8 times the national rate of preventable admissions for congestive heart failure and COPD.

Implementation requires participating providers to maintain 24-hour clinical review capacity and to transmit daily vital-sign data through the state health information exchange. Equipment must function on satellite connections with upload speeds no lower than 1.5 Mbps. Training for home health aides must cover device calibration and escalation protocols approved by the South Dakota Board of Nursing. Pilot sites must report monthly outcome metrics directly to the South Dakota Department of Health rather than through third-party aggregators.

Programs serving the Pine Ridge and Rosebud reservations receive additional scoring for demonstrating tribal health board approval and use of interpreters fluent in Lakota. Unlike Nebraska applications, South Dakota requires demonstration of extreme rural delivery capacity due to frontier county designation.
