Grant Overview
South Dakota limits eligibility for healthy aging grants to organizations that maintain physical offices in at least two of the nine planning and development districts and serve counties where the population aged 65 and older exceeds 18%. Current Census estimates place 19.4% of the state’s 909,000 residents in that age bracket, with the highest concentrations in the west river counties of Butte, Harding, and Perkins.
Applicants must submit letters of support from county commissions and demonstrate capacity to deliver programming in towns with fewer than 2,500 residents. The state does not accept proposals that rely exclusively on telehealth, because 37% of households headed by someone 65 or older report no home internet subscription.
Application requirements include audited financial statements for the prior two years and evidence of at least one staff member holding current certification in chronic disease self-management. South Dakota’s grant review panel additionally requires a letter from the nearest critical access hospital confirming that the applicant has privileges to conduct on-site blood pressure and balance screenings.
Proposals that target the 14 counties with no senior center receive automatic priority points. Unlike Minnesota or Iowa applications, South Dakota requires explicit inclusion of Native American elders living on the nine reservations, with separate enrollment targets and transportation plans coordinated through tribal aging programs.
Fit assessment hinges on whether the applicant already operates under the Older Americans Act Title III or Title VI; organizations without this history must document equivalent service volume for at least 150 unduplicated seniors in the preceding 12 months. Budgets may not exceed 18% for administrative overhead, and all equipment purchases must be sourced through the state’s existing cooperative purchasing agreement.
